{"id":8158,"date":"2022-03-24T09:23:06","date_gmt":"2022-03-24T13:23:06","guid":{"rendered":"https:\/\/gastropedia.pub\/en\/?p=8158"},"modified":"2023-12-29T13:47:23","modified_gmt":"2023-12-29T18:47:23","slug":"endoluminal-functional-lumen-imaging-probe-endofliptm-understanding-the-technology-and-its-potential-uses","status":"publish","type":[121],"link":"https:\/\/gastropedia.pub\/en\/gastroenterology\/endoluminal-functional-lumen-imaging-probe-endofliptm-understanding-the-technology-and-its-potential-uses\/","title":{"rendered":"Endoluminal Functional Lumen Imaging Probe (EndoflipTM): understanding the technology and its potential uses"},"content":{"rendered":"<p class=\"has-text-align-left\">The Endoflip<sup>TM\u00a0<\/sup>is an innovative technique that uses impedance planimetry technology to assess the <strong>distensibility<\/strong> of gastrointestinal organs.<\/p>\n<p class=\"has-text-align-left\">Despite being developed in 2009, its use is still restricted to research environments due to high costs and the need for more evidence for better standardization of the method.<\/p>\n<p class=\"has-text-align-left\">It consists of a catheter that has at its distal end a distensible balloon of 8 or 16 cm (Figures 1 and 2). In this balloon, there are 16 pairs of impedance planimetry sensors, which are capable of measuring the cross-sectional area of a plane of the organ (planimetry) using the electrical resistance (impedance) of the fluid in the balloon.<\/p>\n<p class=\"has-text-align-left\">At the distal end of the catheter, there is also a pressure transducer, which is responsible for measuring the pressure inside the balloon. For this, the catheter is introduced with the patient sedated, usually after upper digestive endoscopy.<\/p>\n<p class=\"has-text-align-left\">With the introduction of Endoflip<sup>TM<\/sup> 2.0 in 2017, a topography system was also associated, which allows the evaluation of esophageal motility (whether absence of waves, whether abnormal retrograde contractions or normal anterograde contractions) &#8211; Figure 3.<\/p>\n<div class=\"wp-block-image wp-image-7956 size-full\">\n<figure class=\"aligncenter\"><img decoding=\"async\" class=\"wp-image-7956\" src=\"https:\/\/gastropedia.com.br\/wp-content\/uploads\/2020\/07\/Imagem3-300x157-1.png\" alt=\"Figure 3: Unaltered exam, presenting esophagogastric junction with normal distensibility and normal anterograde contractions (Dorsey YC et al, 2020).\" \/><figcaption class=\"wp-element-caption\">Figure 3: Unaltered exam, presenting esophagogastric junction with normal distensibility and normal anterograde contractions (Dorsey YC et al, 2020).<\/figcaption><\/figure>\n<\/div>\n<h4 class=\"wp-block-heading\"><\/h4>\n<h2 class=\"wp-block-heading\"><strong>The potential applications of the method are:<\/strong><\/h2>\n<h3 class=\"wp-block-heading\"><strong> 1. Evaluation of dysphagia and achalasia<\/strong><\/h3>\n<ul>\n<li>Highlight in those patients with clinical suspicion of achalasia, but diagnostic doubt due to normal relaxation of the esophagogastric junction (EGJ) in manometry exam;<\/li>\n<li>Usefulness in patients who cannot perform manometry due to not tolerating the discomfort of the probe (Endoflip<sup>TM<\/sup> is performed sedated);<\/li>\n<li>Distensibility index of the EGJ &gt; 3 mm<sup>2<\/sup>\/mmHg and anterograde contractions suggest normality (Figure 3);<\/li>\n<li>Distensibility index \u00a3 1.6 mm<sup>2<\/sup>\/mmHg of the EGJ, as well as absence of contractions (figure 4) or repetitive retrograde contractions (figure 5) suggest achalasia.<\/li>\n<li>In cases of manometric diagnosis of EGJ flow obstruction, the Distensibility Index of the EGJ &lt; 2 mm<sup>2<\/sup>\/mmHg is associated with better symptomatic response to therapies similar to achalasia, while values &gt; 3 mm<sup>2<\/sup>\/mmHg are favorable to conservative follow-up.<\/li>\n<\/ul>\n<div class=\"wp-block-image wp-image-7957 size-full\">\n<figure class=\"aligncenter\"><a href=\"https:\/\/gastropedia.com.br\/wp-content\/uploads\/2022\/11\/endoflip4.png\" data-rel=\"penci-gallery-image-content\"  data-rel=\"penci-gallery-image-content\"><img decoding=\"async\" class=\"wp-image-3842\" src=\"https:\/\/gastropedia.com.br\/wp-content\/uploads\/2022\/11\/endoflip4.png\" alt=\"\" width=\"300\" height=\"226\" \/><\/a><figcaption class=\"wp-element-caption\">Figure 4: Esophagogastric junction with reduced distensibility and absence of contractions, suggesting Type I Achalasia (Dorsey YC et al, 2020).<\/figcaption><\/figure>\n<\/div>\n<div class=\"wp-block-image wp-image-7958 size-full\">\n<figure class=\"aligncenter\"><img decoding=\"async\" class=\"wp-image-7958\" src=\"https:\/\/gastropedia.com.br\/wp-content\/uploads\/2020\/07\/2.png\" alt=\"esophagogastric-junction-with-reduced-distensibility-type-3-achalasia.jpg\" \/><figcaption class=\"wp-element-caption\">Figure 5: Esophagogastric junction with reduced distensibility and repetitive retrograde contractions, suggesting Type III Achalasia (Dorsey YC et al, 2020).<\/figcaption><\/figure>\n<\/div>\n<h3 class=\"wp-block-heading\"><strong>2. Evaluation in eosinophilic esophagitis<\/strong><\/h3>\n<ul>\n<li>Identify esophageal distensibility, in order to identify fibrostenotic narrowings that are not always well evaluated by endoscopy.<\/li>\n<li>Potential benefit in patients who persist with dysphagia despite histological remission, possibly guiding possible dilations.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\"><strong>4.
